There is one optional note at the end, and nothing else to fill in. No email address is required to see the result, and contact details are only asked for if you choose to turn it into a brief.
This is a short read on delivery conditions. It is not a programme assurance review and it does not replace one. If the programme is large or already in trouble, a proper independent review is the right next step.
